To familiarize the patient with the procedure
To establish contact between the patient and physicians, who will answer questions the patient has regarding the procedure, alternative treatments, post procedure care and follow-up.
The patient will be asked to fill out a questionnaire to gather the patient’s health status, health history, and insurance information.
